The Cardiac Network is pleased to offer a bite sized webinar presented by community pharmacist, Kevin Western, who has been instrumental in promoting the use of DMS within the East of England. Kevin will explain the process, how to refer and demonstrate the benefits to both the patients safety and wellbeing as well as reducing pharmaceutical waste.

 

The NHS Discharge Medicines Service is an established, essential, commissioned service provided by all community pharmacy contractors.

The service aim is to ensure better communication of changes to a patients medication when they leave hospital or are discharged from a provider and to reduce incidences of avoidable harm caused by medicines.  It is an integrated approach to supporting patients with medicines reconciliation and optimisation following discharge, ensuring clear and identifiable links between community pharmacy, NHS trusts and PCN member practices. DMS is available for all patients, regardless of which long term condition they have.

 

DMS referral allows patients access to expert advice in a timely manner to give them autonomy over their treatment and enables swift management of adverse side effects, gives patients an improved understanding of often multiple medicine changes post discharge, ensures medicines are stopped as intended, and reduces hospital readmissions because of medication issues.
