BCF - Intermediate Care Workforce Pilot Sites Webinar – Harrogate & Hounslow

We are pleased to invite you to an upcoming webinar focused on sharing learning and good practice from two workforce pilot sites.

During this 90-minute session, we will hear from teams in Hounslow and Harrogate, who will each present innovative approaches to improving patient flow and care coordination.

What to Expect:

Hounslow - Intermediate care – Nurse Pilot- 

Hounslow will showcase a case study of their model on pilot integrates nursing roles into Intermediate Care to fill a critical gap in community and discharge services. The project demonstrates how two full-time nurses embedded within the Community Recovery Service have reduced hospital admissions by 27% and empowered staff and patients alike. They will showcase how embedding clinical leadership within reablement services improves safety, clinical quality, and patient experience—especially for those with complex health needs.

Harrogate - Proactive Identification & Tiering in the Emergency Department-
The Harrogate team will share insights from their pilot of a tiering system within the Emergency Department. This proactive approach aims to better identify, and route patients based on their level of risk and need. The presentation will cover the design of the model, its practical implementation, staff collaboration, and the early impact on discharge planning and resource allocation.

This is a valuable opportunity to explore practical, frontline innovations and participate in Q&A discussions with the teams.
