Join colleagues from health, care, and voluntary sector to explore examples of what’s working well to enable the right care at the right time in the right place over winter. Be part of this sharing and learning to hear from system colleagues and providers: Be inspired and take away tangible resources and quality improvement ideas to put into action in your patch.
The Regional Primary Community and Personalised Care Team works across three areas:
- proactive and preventative care,
- urgent and acute deterioration and
- long term conditions and intermediate care
We are privileged to work with people in all sectors across the south west, providers, commissioners, local authorities and the voluntary sector. We wanted to take this opportunity on a Tuesday lunchtime to showcase some of the amazing work that is taking place across our region. The session will provide an opportunity for you to see how teams utilise an integrated neighbourhood way of working to better support people and communities to live well at home. As we head towards winter, we want to ensure systems and teams working on the ground are in the best place possible to manage and support our communities
