This training is designed for multi-agency professionals who play a vital role in supporting children and young people with complex health needs. In this hour, we’ll focus on the Children and Young People’s Continuing Care (CYPCC) Framework—what it is, how it works, and how we collaborate to make informed decisions.

Session Objectives

By the end of this session, you will:

  1. Understand the CYPCC Framework – its purpose, principles, and how it supports holistic care planning.
  2. Explore the referral and assessment processes – including how assessments are completed, what information is gathered, and the tools used.
  3. Learn how decisions are reached – the role of multi-agency panels, evidence-based decision-making, and ensuring transparency and fairness.
  4. Have the opportunity to ask questions – we’ll dedicate time at the end for discussion and clarification.

The CYPCC Framework ensures that children and young people with complex needs receive care that is personalised, equitable, and sustainable. 

 

NHS England is Hosting this Training in collaboration with the 7 ICBs In the South West Region. The sessions will be delivered by experienced professionals from across the ICBs that work within CYPCC.
