Regional HCL Diabetes Transformation Workshop: PIFU and Outpatient Innovation [In-person event]

The East of England Diabetes Clinical Network, in collaboration with Health Innovation East and Getting It Right First Time (GIRFT), is hosting a regional workshop focused on Patient Initiated Follow-Up (PIFU) and outpatient transformation in diabetes services.
As the HCL Diabetes Transformation Programme enters Year 3 of 5, this interactive event will bring together consultants, clinical leads, diabetes specialist nurses, outpatient leads and transformation leads to explore how services can adapt to increasing demand, improve patient experience and support more sustainable outpatient models.
The workshop will include:
- Updates on the HCL Diabetes Transformation Programme and outpatient redesign priorities.
- Shared learning and best practice from across the East of England.
- Insights from the Sheffield PIFU model and practical implementation approaches.
- Outpatient transformation and PIFU implementation - Jackie Elliot PhD, FRCP - Clinical Lead for Diabetes, Senior Clinical Lecturer & Honorary Consultant, Sheffield Teaching Hospitals NHS Foundation Trust
- Risk stratification model within diabetes pathways - Dora Deane -Deputy Operations Director, GRaDE, Sheffield Teaching Hospitals NHS Foundation Trust
- Facilitated discussion focused on overcoming barriers, identifying opportunities and agreeing practical actions for local implementation.
Designed as a discussion-led workshop rather than a presentation-focused event, attendees will have the opportunity to collaborate with colleagues from across the region, share challenges and learn from successful approaches already being implemented within diabetes services.
The workshop is intended for diabetes clinical leads, consultant diabetologists, diabetes specialist nurses, service and operational leads, transformation leads, HCL programme colleagues and other healthcare professionals involved in diabetes service delivery, outpatient transformation and pathway redesign across the East of England.
