Carers Week 2026- Hospital to Community Session

We are delighted to invite you to Carers Week 2026.

This year Carers Week will take place between 8th – 14th June 2026 and the theme this year is: “Building Carer Friendly Communities”

We are delighted to be joined by fabulous guest speakers throughout the week, so please join us as we shine a light on the fantastic work happening across our providers, systems and regions.

Come and join Laura Green, as she shares the Kingston and Richmond NHSFT Carers’ Clinical Liaison Service. The service was established in 2022 in response to Health & Care Act Section 19.

The ambition is to improve & measure carer experience of hospital discharge across the two local systems- Kingston Upon Thames and Richmond Upon Thames & Wandsworth.

The session will:

  • Provide an overview of the Carers’ Clinical Liaison Service at Kingston and Richmond NHSFT
  • Explore how unpaid carers are identified and engaged within hospital settings
  • Demonstrate how carers are supported during discharge planning and transition to community‑based care
  • Highlight the role of partnership working withcommunity services, primary care and local carer organisations
  • Share learning and insights that can support safer discharge, reduced readmissions and improved carer experience

This session will be chaired by Ros Spinks, Senior Quality and Patient Safety Manager at NHS England, London Region.
