Part 1: Healthier Lives: Empowering people with learning disabilities to take charge of their well-being

Join us for a webinar showcasing how Hertfordshire Learning Disability Nurses are leading initiatives to improve health outcomes for people with learning disabilities in the community. We will introduce the 'Stay Healthy at Home' checklist, designed to support a 365-day approach to health promotion, ensuring well-being extends far beyond the annual health check.

Find out how our Health Equality Nurses (HENs) are addressing health inequalities, increasing cancer screening uptake, and reaching individuals who may otherwise miss essential health services. We will also introduce the 'Healthy Lifestyles' workshop series - a tailored programme designed to empower people with learning disabilities and their carers with practical knowledge on staying healthy, enhancing well-being, and making informed lifestyle choices.

Speaker:

Hilary Gardener is the strategic Liaison nurse for Primary Health in Hertfordshire and is part of the Adult Care services learning disability nursing service.

She is passionate about establishing pathways that help reduce the risk of avoidable deaths in people with a learning disability through the principle of the triangle of health being a partnership between the person, the people who know the person best and the health professionals.
