Join representatives from NHS England, local NHS organisations and vendors of specialist solutions for a day looking at how digital technology can support the effective delivery of personalised care.

Why you should attend?

Personalised care will be a central priority of the NHS Long Term Plan, due to be published in November 2018.

Personalised care means people have choice and control over the way their care is planned and delivered, based on ‘what matters’ to them and their individual strengths and needs. This happens when people are supported to stay well for longer and makes the most of the expertise, capacity and potential of people, families and communities in delivering better outcomes and experiences when unwell. The result is better health and well-being outcomes for people, plus more effective and joined-up services.

To achieve this at scale, technology will be at the heart of delivering and driving the Long Term Plan and person centred care. Local digital provision needs to be supported to do this by national professional bodies in a systematic and coordinated way.

This one-day event will allow local leaders to understand the vision and what practical steps they can take to implement a major growth area in the NHS.

What will you get out of the day?

There will include a range of opportunities to learn about how the digital opportunities in personalised care fit into the wider NHS digital strategy, and how local organisations can best make use of the digital tools on offer.

There will be opportunities to learn more about the range of digital solutions on offer, including the first opportunity to discover which ones have been selected by NHS England as suitable for either e-marketplaces for care; or personalised care and support planning. You’ll be able to spend time discussing your requirements with IT suppliers.

Who should attend?

The day is aimed at:

  • Personalised Care leads
  • Commissioners from STPs / ICSs / CCGs / local authorities
  • Digital leads in STPs / ICSs / CCGs / local authorities
