Please join our next Digi-bites session to find out more about the London Universal Care Plan and hear from Nicholas Tigere, Head of the Universal Care Plan Programme and Tomas Ince, Senior Clinical Transformation Manager, as they explain more about what the Universal Care Plan is, how it works and the impact it has had in the London region. Hosted by your Digital Transformation colleagues in the East of England these free education sessions could offer valuable insights. 

The Universal Care Plan (UCP) is an NHS service that enables every Londoner to have a digital personalised care plan which, thanks to recent exciting developments, they can now create and edit themselves via the NHS App. These care plans are digitally shared with health and care professionals across the capital, helping to ensure Londoners get the joined-up care they need and want. The UCP can be accessed via the London Care Record, some local Electronic Patient Record systems and via the UCP web portal. It includes information such as what’s important to a person, their preferences or wishes about their care and what support they need.
