NHS England is running a Patient Activation Measure (PAM) and Supported self-management (SSM) training session.  The aim of the session is to share learning on how to successfully implement and scale up use of the PAM to tailor supported self-management approaches. This training is open to;

  • Practitioners using PAM licences through the NHS England Supported Self-management programme
  • Personalised Care leads from STPs/ICSs, CCGs who want to scale up the implementation of PAM & SSM
  • PCNs or social prescribing schemes using (or intending to use) PAM licences
  • Any other CCGs, providers, voluntary sector sites interested in using PAM

Tailoring support and self-management interventions to an individual’s needs can support people to develop their knowledge, skills and confidence to manage their long-term conditions more effectively, improve their health outcomes, ensure a better experience of care and reduce unplanned care admissions.

The day will give an overview of the context including how this links to the Long Term Plan, the history of using PAM in the NHS, what the PAM is, what it means for different parts of the NHS and what difference using PAM will make to the system, how to get started, how to tailor approaches and how to engage at each level.
