Co-ordinated frailty care for better outcomes

Hosted by Adrian Hopper, GIRFT clinical lead for geriatric medicine, this webinar will discuss GIRFT’s new guidance to support co-ordinated frailty care for better outcomes, co-badged by the British Geriatrics Society (BGS).

Population growth in the number of people aged over 85 means all healthcare professionals, regardless of their specialty, need the knowledge and skills to deliver effective care to older people. This guidance offers key tools, actions, practical advice and checklists to support an optimal system-wide approach, ensuring older people with frailty receive timely access to care in the right setting. 

Contributors to the guidance Dr Helen Skinner (GIRFT specialty adviser and chief medical officer, stroke consultant and geriatrician at University Hospitals of Morecambe Bay NHS Foundation Trust) and Dr Tom Bartlett (consultant geriatrician at University Hospitals Dorset NHS Foundation Trust, and the British Geriatrics Society vice president for clinical quality), will discuss their experiences of frailty care in an integrated trust,  and how they have worked with partners to embed a system approach to delivering optimal care for older people living with frailty.
