During this 90-minute session, we will hear from teams in Tower Hamlet and Sussex, who will each present innovative approaches to improving patient flow and care coordination.

Tower Hamlets - Integration of Physiotherapy into Adult Social Care & Foot Health Homelessness Project

What to Expect:

Tower Hamlet will show case their case study on how integrated physiotherapy services into Reablement and Initial Assessment Team (front door of ASC) and foot health clinics supporting people experiencing homelessness. By combining adult social care with proactive physiotherapy, the project addresses complex needs through a community-first approach. The session will detail how workforce planning, caseload reviews, and pathway stratification were used to deliver more inclusive, targeted care—and how these strategies are driving wider service transformation across the system.

Hounslow - Intermediate care – Nurse Pilot

What to Expect:

Hounslow will showcase a case study of their model on pilot integrates nursing roles into Intermediate Care to fill a critical gap in community and discharge services. The project demonstrates how two full-time nurses embedded within the Community Recovery Service have reduced hospital admissions by 27% and empowered staff and patients alike. They will showcase how embedding clinical leadership within reablement services improves safety, clinical quality, and patient experience—especially for those with complex health needs.


This is a valuable opportunity to explore practical, frontline innovations and participate in Q&A discussions with the teams.
