Warwickshire - Community Recovery Services (CRS) – Predictive Stratification

What to Expect:

Warwickshire Community Recovery Services will be showcasing their case study which focuses on improving patient care and workforce planning in intermediate care through predictive stratification. The team developed a data-driven tool that helps stratify patients by care needs and forecast the workforce required to support them. This webinar will explore how this model improves service planning, resource allocation, and patient outcomes across intermediate care.

Devon – Stratified Workforce Planning

What to Expect:
Devon will be showcasing their pilot project which focuses on improving how care is delivered by aligning staff skills to patient needs. The team has developed a clear, data-driven tool that stratifies patients based on the complexity of their care requirements. This approach helps identify which cases can be safely managed by Band 4 Assistant Practitioners, reducing pressure on nurses and therapists.

The tool draws on two familiar measures — the Barthel Index and the Rehabilitation Complexity Score — to support safe delegation decisions. Although still in its pilot stage, early feedback suggests the model is making planning more straightforward and boosting staff confidence in task allocation.

This model lays the groundwork for more efficient and responsive care by enabling better use of the full workforce.
