'Learning from deaths' – guidance development day for NHS Trusts

The ‘Learning from Deaths’ programme was established to ensure that the recommendations contained in the CQC’s ‘Learning, candour and accountability’ report are implemented in a clear and structured way. 

Part of this programme is focused on recommendation 3 of the report. This highlights the need to improve engagement by setting out what families and carers can expect from the investigation process. The CQC report states that guidance needs to be developed for Trusts in partnership with families/carers. The aim of this guidance will be to ensure timely, transparent and compassionate contact with families and carers who’ve lost loved ones in NHS commissioned care.

 NHS England is holding two events to inform the development of this guidance. These will be run on two consecutive days, and we’re inviting you to be involved in the second day:

  • 1 November – This day will primarily involve family members and carers with lived experience of the review/investigation process. We’re also inviting advocate organisations/charities, and some health professionals will be involved.
  • 2 November 2017 – This day will primarily involve health professionals, (with some family members and carers also present). The day will focus on the key issues already identified in the CQC report; what good practice should look like; how we can embed good practice across NHS Trusts; and what support and/or training would be needed.

This second day is primarily for people working in NHS Trusts who are involved in investigations, including:

  • Medical Directors
  • Patient Experience Leads
  • Chief Investigating Officers who investigate deaths in the NHS
  • Chaplaincy services

Both events will be accessible online, if you’re unable to attend in person. We will be webcasting live on both dates, with online discussion forums to enable people to participate remotely. 

Our intention is to co-produce the guidance, and for it to be published in early 2018.
