Leeds is one of 3 Intermediate Care Frontrunner pilot sites, working with NHS England.
Leeds has a vision to create a ‘ A sustainable, person-centred, home-first model of intermediate care across Leeds that is joined up and promotes independence’. This vision is becoming a reality through the ‘HomeFirst’ Programme – a multi-year partnership transformation programme which will ensure over 3000 people in Leeds have better outcomes every year. The programme is made up of five interdependent projects. This webinar will explore the detail of the ‘system visibility and active leadership’ project. This project is embedding a culture of data-driven decision making with the visibility to see how people are moving through our health and care system, working together to help more people return or stay at home.
This webinar will consider the system visibility tool– how it was designed, built and continues to be maintained, including the technical and infrastructure barriers that were overcome to do so. It will also cover how the tool is used, with examples from day to day operational pressures to service re-design, and how the culture of data driven active leadership is becoming a reality across the partnership.
