Managing Heart Failure @Home and in the community

Living with heart failure is becoming increasingly common. In the UK, heart failure affects approximately 920,000 people with 200,000 new cases annually, and these numbers are increasing. It is now estimated that diagnosed cases are predicted to nearly double by 2040.


The webinar will showcase projects that have implemented the managing heart failure at home approach and multidisciplinary teams (MDTs) in the community to support reduction in admissions and support primary care management of heart failure.  

This webinar is for all colleagues with an interest in heart failure patients across primary, secondary and community care as well as system and network colleauges. Please share this invite with your ICB/PCN clinical and non-clinical colleagues and others who may be interested in this topic.
