Population Health Management is an emerging agenda. No area could confidently claim to be the ‘finished article’ and practice will continue to evolve. New sources of insight will be produced; new applications will be found; new ways of translating information into action will be devised.

This environment is ripe for mutual learning. As STPs/ICSs develop their practice, there will be opportunities to share what they did, why they did it, what resulted and what they’ve learnt. Done openly and honestly, this gives other areas a chance to benefit: to replicate good practice and avoid pitfalls.

With this principle as our core value the Population Health Programme Board would like to develop a Community of Practice.

Participants will work though the practicalities of approaches and as individuals and organisations commit to mutual sharing and learning. This might mean presenting on specific areas of practice; it might mean committing to Task and Finish groups on topics that need more detailed examination; it might mean bringing in examples and presenters from other areas, exploring national and international evidence. Most of all this will mean that we work collaboratively to continuously improve and transform health & care through shared learning and practice.
