National Frailty Conference (please scroll down for booking link)

Frailty is often described as the most problematic expression of population ageing.  Better recognition of the condition also offers significant opportunities to improve care and support for older people.  This conference will explore ways we can identify people living with frailty and improve their care to support wellbeing and independence.  It will consider interfaces across community, social care, primary and acute healthcare. 

Attendance is free for health and care practitioners and commissioners.
