• The purpose of the packs and how to use them
• A worked example of a CCG from your region to illustrate: - CCG inequalities in Unplanned Hospitalisations - CCG Improvement and Assessment Framework (IAF) Health Inequalities Indicator 106a: Inequality in Unplanned Hospitalisations for Chronic Ambulatory Care Sensitive and Urgent Care Sensitive Conditions for 2016/17. Inequalities in Psychological Therapies - CCG Indicators of Equity for Increasing Access to Psychological Therapies (IAPT) for 2016/17
• Knowing your population groups
• An opportunity to discuss with regional colleagues key areas of inequalities or concerns in your CCG; regional support to address health inequalities and what that support might look like; capture notable practice: 'what's making a positive difference?'
