Personalised Care and Support Planning Study Day

Personalised care and support plans (PCSPs) sit at the heart of a personal health budget (PHB), it's essential that PCSPs are correctly executed and the individual is supported to develop their plan as much as required. Individuals should have the opportunity to develop a good care and support plan even if they then decide to choose to manage their PHB through a notional budget. 

Who should attend?

This workshop is for anyone who is directly involved in developing personalised care and support plans.

Programme for workshop

This event will cover the following areas:

• The key features of PCSPs and what 'good' looks like
• Tools and resources to enable staff to have a 'different' conversation as part of CHC case management and understand how to record it
• Developing outcomes with people, including one page profiles
