Redesigning Heart Failure Services across Greater Manchester

Introduction
Over the last 18 months, the Cardiac Network within the Greater Manchester and Eastern Cheshire Strategic Clinical Networks (GMEC SCN) has been working on an improvement programme encompassing 5 project groups, one of which is the delivery of a NICE aligned pathway for heart failure.
We would like to provide the opportunity for teams to come together across Greater Manchester to discuss and plan ways for us to work together to improve cardiovascular health.
This area includes work being led by Health Innovation Manchester (HInM) for a new digitally-enhanced service for people living with heart failure in Greater Manchester, using data from existing implantable devices to transform care.
The overall aim of our project is to improve quality of life and outcomes for heart failure patients across Greater Manchester, supporting better diagnosis, improved treatment and management across a full patient pathway.
The purpose of the event is to present our heart failure outputs which comprises of the following:
- NT-Pro BNP Testing Comms piece for primary care
- Greater Manchester Heart Failure Care Plan for patients
- Acute and Community Heart Failure dashboards
- Greater Manchester Competency Framework for heart failure nurses
- ‘As is’ service maps for the localities across Greater Manchester
- Share findings from the HInM “Smart Hearts” heart failure project
These outputs have been produced alongside various stakeholders and are outcomes put together by members of our Community Heart Failure Working Group.
Aimed at key stakeholders including
Patients, carers, commissioners, charities, third sector organisations, consultants, practice nurses, heart failure nurses, clinical leads, primary care and GPs.

The Greater Manchester Heart Failure work programme is being delivered in collaboration with third parties, including, but exclusive to, the pharmaceutical industry and third sector organisations, who bring their expertise and insights for the benefit of patients and the health system.
Parking
This is complimentary however all delegates need to enter car registrations onto the iPad which is outside the room where this event is taking place. Then and only then will costs be waivered.
