Redesigning Stable Chest Pain Services across Greater Manchester

Introduction

Work on cardiovascular health improvement across Greater Manchester and Eastern Cheshire (GMEC) has been continuous over the last few years with teams working on different parts of the cardiovascular disease (CVD) pathway.  Over the last 18 months, the Cardiac Strategic Clinical Network (SCN) has been working on an improvement programme encompassing 5 project groups, one of which is the delivery of a NICE aligned pathway for stable chest pain.

We would like to provide the opportunity for teams to come together to discuss and plan ways for us to work together to improve cardiovascular health across GMEC.

Our morning session will have a specific focus on Improvements and Innovations in Cardiology Care across Greater Manchester.

The aim of our meeting is to

• Understand how we currently deliver care for stable chest pain patients across Greater Manchester; highlight areas of best practice; understand the data; champion examples of innovation.
• Aligning our system and services to NICE; our journey to harmonise and modernise chest pain assessment through development of a best practice framework and sustainable pathway of care.
• Articulate the tools and processes to support implementation of a future service; e-referral, standards and pathway review.
• Provide an opportunity for delegates to discuss how we implement the future model of care; What are the enablers to delivery? What are our watch outs?
