Referral Optimisation Network: Developing an elective approach to neighbourhood health
About this event:
Join us to learn how respiratory elective care for adults in Birmingham is being transformed through developing a neighbourhood health approach for planned care.
In this primary care-led service, patients are referred to the Respiratory Specialist Review Service hub where a multi-professional team work in partnership as peers in a learning environment to provide care including early access to specialist input, rapid and local diagnostics, and treatment where clinically appropriate.
In this session, we will share how clinical partnerships have been pivotal in the design and delivery of holistic and personalised care, along with our early thinking on how this emerging elective neighbourhood health model could be scaled at ICB level — supporting the ambitions of the 10-Year Health Plan to shift care delivery from hospital to community settings.
This initiative is being led by the national Referral Optimisation team.
Audience: The Referral Optimisation Network brings together NHS clinical, operational and commissioning managers and leads and other relevant NHS staff involved in the improvement and delivery of referral optimisation.
