Addressing health inequalities and ensuring equitable access to healthcare are fundamental responsibilities for all stakeholders in the health and social care sectors.
Cornwall and Devon demographics predominantly resemble an ageing population with high levels of employment in the agricultural industries. Rural and coastal communities face unique challenges in accessing services. It became evident that many organisations were working in silos towards the same CORE20PLUS5 goals.
This project is a collaborative, cross-border initiative focused on multimorbidity, uniting primary, community, and secondary care, public health, VCSE, academia, agricultural industries and communities plus governmental bodies. The aim is to enhance the health and well-being of our rural communities by promoting place-based care through a sustainable farming health hub model.
You are invited to a virtual showcase on Wednesday 17th September, 9.30 – 11.30am which will include the evolution of the model, funding, metrics, research and next steps.
