HEAR US: Enabling a purposeful post incident debrief
Join us to learn how the HEAR US and I am HEARD tools were co-produced and implemented in partnership with people with lived experience, patients, carers and healthcare professionals to improve post-incident debriefs and reduce restrictive practice.
Hear how the HEAR US framework has been adopted to support meaningful, person-centred debriefing, helping organisations capture learning and improve experiences following incidents. The session will also showcase I am HEARD, a co-produced checklist that empowers patients, carers and loved ones to understand what to expect after an incident and play an active role in the debrief process. Together these tools are strengthening partnership working, improving post-incident learning and supporting more transparent, person-centred care.
