This is a follow-up event to support building a Standardised Discharge approach across all Spinal Units in relation to CHC

 

• Key Objectives of proposed workshop:

  • Review of actions since March 2020 workshop
  • Building a Standardised Discharge approach across all Spinal Units in relation to NHS CHC
  • Ensuring the NHS CHC assessment is completed at the right time and right place involving those that know the patient well
  • What a good discharge checklist looks like for Spinal Injury Units
  • Explore a standard discharge process in relation to NHS CHC & Spinal Injury units
