Join us for an informative and practical webinar focused on strategies to avoid unnecessary hospital admissions, particularly for people with learning disabilities. This session will explore the proactive approaches to care used within BLMK, early identification of health deterioration, and effective use of community resources. We’ll also highlight real-life case studies and tools such as hospital passports, and reasonable adjustments in healthcare settings.
