Understanding Personalised Care – an event for provider organisations

Personalised Care is one of the five major, practical changes to the NHS that will take place over the next five years, as set out in the NHS Long Term Plan.

Personalised Care will benefit up to 2.5 million people by 2024, giving them the same choice and control over their mental and physical health that they have come to expect in every other aspect of their life. 

Personalised care means people have choice and control over the way their care is planned and delivered and is based on ‘what matters’ to them and their individual strengths and needs.  Personalised care takes a whole-system approach, integrating services around the person including health, social care, public health and wider services, and provides an all-age approach from maternity and childhood right through to end of life, encompassing both mental and physical health and recognises the role and voice of carers. It recognises the contribution of communities and the voluntary and community sector to support people and build resilience. Working closely with partners, the NHS will roll out personalised care to reach two million people by 2023/24 and then aim to double that again within a decade. 

The NHS Long Term Plan says personalised care will become business as usual across the health and care system and we are inviting people from provider organisations to join us for a workshop to learn more about how the Universal Personalised Care model confirms how this will be done.

This workshop is for decision makers and influencers in transformation, finance, contracting, commissioning, clinical leadership etc who will benefit from understanding how personalised care can support local health and care strategies.  The workshop will bring to life the different components of the Universal Personalised Care model including personalised care and support planning; social prescribing; shared decision making; personal health budgets; co-production; finance and commissioning; and more.
