- Identify issues and opportunities for improvement
- Target patients who have rising risk levels or are impactible
- Predict and evaluate impact
Morning Workshop 1 - What intelligence is needed to achieve the triple aim objectives for the following priorities (please select upon registration):
- Group 1 - Frailty
- Group 2 - Long term conditions
- Group 3 - Maternity and first 100 days of life
*Note - all groups will explore the priority from a whole system and entire patient pathway (prevention to palliative care) perspective; mental health and social care will be incorporated into all groups
Afternoon Workshop 2 - population health intelligence enablers and challenges (please select upon registration):
- Group 1: Information Governance
- Group 2: Data management (data storage, access, linkage, quality and processing)
- Group 3: Intelligence services (managing in house services and suppliers)
- Group 4: Culture (of evidence based decision making, and improving intelligence)
