Working to improve access to general practice through social prescribing and placed based working

Social prescribing and personalised care can help to alleviate demand for GP services and create new opportunities for place-based working

To increase knowledge and understanding about Social Prescribing Link Workers are supporting improvements in GP access, including on the day access. 

Understand how the three personalised care roles, including Care Coordinators and Health and Wellbeing Coaches work together to improve access. 

Consider opportunities to develop place-based working and greater integration of services to reduce inequalities in access to health services.

• Explain how the wider personalised care roles can help  
• Showcase examples of best practice relating to access and place based working
